How To Access Your Medical Records

Methodist Health System provides release of health information for patients who request medical records from Methodist Health System hospitals in accordance with all state laws and regulations. See below for the ways in which you may access your medical records.

1. MyChart Patient Portal
   • MyChart is a user-friendly tool for patients to quickly access their medical records electronically 24 hours a day! Please click HERE to learn how to register and use your MyChart Patient Portal.
2. Emailing your Authorization To Disclose Health Information Form to MHSROI@MHD.COM.
   • To request copies of your complete medical records, please download, complete and sign the Authorization to Disclose Health Information.

   • Please note that if you send to this email address that you may be sending your Protected Health Information via unencrypted email, which may not be a secure way to transmit such information. Although it is unlikely, there is a possibility that information that you send to and receive from this inbox can be intercepted and read by other parties besides the person to whom it is addressed. You have been warned of and accepted the risks associated with the unsecure transmission.
   • Once you have completed and signed the Authorization to Disclose Health Information Form, you must email it to MHSROI@MHD.com. Please note that there may be a fee for processing your medical records request(s). However, there is no charge to have records sent directly to another health care facility or physician for continuation of care.
3. Going in-person to one of our medical records offices at the below facility:

   • Methodist Dallas Medical Center –1441 N. Beckley Avenue, Dallas, Texas 75203; 2nd floor Schenkel Tower between radiology and administration.

   • Methodist Charlton Medical Center – 3500 W. Wheatland Road, Dallas, Texas 75237; 1st floor near cafeteria.

   • Methodist Mansfield Medical Center – 2700 E. Broad Street, Mansfield, Texas 76063; 3rd Floor Women’s Center across from HR.

   • Methodist Midlothian Medical Center – 1201 E. Us-287, Midlothian, Texas 76065; 1st floor across from administration.

   • Methodist Southlake Medical Center – 421 E Estate Hwy 115, Southlake, Texas 76092; 2nd floor between administration and ICU.

   • Methodist Richardson Medical Center – 2831 E President George Bush Hwy, Richardson, TX 75082; 1st floor next to administration on Bush/Renner Campus.
