We’re here to make your visit as smooth as possible. Fill out your
paperwork ahead of time and learn what to expect at your appointment.
Scheduling an appointment
- Please call our office at 469-204-5620, after selecting your language (English:
1 or Spanish: 2), then press 1 for new patients, or 2 for established
patients to reach the corresponding scheduler..

If you are being referred by a physician, have the physician’s name and phone number ready so that we can
have your medical records faxed over.

If you are a self-referral, fax copies of your latest laboratory results, radiology reports, pathology
results, and any office notes from your last doctor’s visit to 469-204-5626,
or let the office know that you will bring your records to your first visit.
- Please note: It is important to know the patient’s condition for
scheduling.
What to bring

If a new patient, a copy of your disc from your last CT and/or MRI: Contact the office/hospital where you had the scan and request to pick
up a copy before your visit. We do not recommend mailing the disc to the
office, as we do not always receive it before your visit.

Current insurance card and driver’s license or picture ID: If your insurance is an HMO, contact your primary care physician to request
an authorization and referral to be faxed to our office. If your insurance
requires an authorization and this is not obtained, it could postpone
your initial visit. Contact your insurance directly to ensure our providers
are in-network with your insurance, and be prepared to make any cost-share
(deductibles, coinsurance, and/or copayments) at the time of check-in.

New patient paperwork: When you make an appointment, we will email you a fillable PDF form.
We encourage you to print and complete it beforehand, arriving 15 minutes
early. If you cannot complete the form before you arrive, then please
plan to arrive 30 minutes early to fill out the paperwork in the office.

A list of medications, pharmacy phone number, and contacts for any doctors
caring for you: Include medication dosages or bring your medication bottles to your visit.
Please provide each doctor’s full name and contact number in case
your physician needs to contact them directly.
Parking
- Parking is located on the side of Methodist Richardson Cancer Center or
behind the Physician Office Building.
What to expect
- Our average office wait time is up to 15 minutes, but please keep in mind
that there are times when our office is busier than normal. We want every
patient and family to have all of their questions answered and receive
our complete attention.
- Expect your first office visit to last at least an hour once in a patient
exam room.
- The medical assistants will get your current weight, blood pressure, and
temperature and enter your medical history, surgical history, medications,
and allergies.
- Depending on your physician, a surgical resident or surgical fellow may
come to speak with you to review your health information while the doctor
is viewing your current CT and/or MRI images.
- After the surgeon speaks with you, you may need more tests or be referred
to an outside physician for surgery clearance. The medical assistants
will call you the following day with your testing appointment times. If
you are referred to an outside physician, we will fax the referral directly
to their office, and their office will call you to set up an appointment.
- When you and the surgeon decide to proceed with surgery, the risks and
benefits of your surgery will be discussed in detail. The surgery coordinator
will give you a date for your surgery along with pre-op instructions.
- You will receive separate calls for surgical cost estimates. One call will
come from our staff with estimated professional service charges. The other
call will come from the hospital Financial Services for the hospital’s
portion of the bill.
- We strive to collect the estimate in full three days prior to the surgery.
