In order to provide excellent patient service, please be prepared for your appointment.
How to Prepare for Your Appointment
Bring completed new patient forms.
Arrive 15 minutes ahead of your scheduled appointment time.
Bring a current list of all medications you are taking.
Please bring your insurance card and your state identification card to each visit.
Special note: Copayments, deductibles, and coinsurance must be collected at the time of services are rendered. We accept cash, personal checks, Visa, MasterCard, Discover, and American Express.
